NEW: COVID-19 Office Plan


This page will let you know what to expect before and during your upcoming visit to our office. We are following guidelines set by Yale New Haven Health as well as the State of Connecticut and the Centers for Disease Control and Prevention in order to reduce the spread of disease. Please be patient with us during this trying time—we are doing our best to keep you and all of our patients safe and healthy.

 

Two days prior to your visit:

In addition to an automated appointment reminder call, you will be receiving a phone call from someone in our office 2 days before your scheduled appointment to review your personal information and to review our new safety measures and instructions for your visit.  

 

Please keep in mind that if this process is not performed by the day prior to your visit, your appointment might need to be rescheduled.

 

First, you will be asked to confirm you name, address, phone number, insurance, and Primary Care Provider.

 

Next, you will be asked series of questions regarding COVID-19 exposure:

  • Have you have traveled outside of Connecticut in the last 2 weeks? 
  • Have you been in close contact with anyone from outside of Connecticut in the last 2 weeks?  
  • Have you tested positive for COVID-19 in the last 2 weeks? 
  • Have you knowingly been exposed to anyone with COVID-19 in the last 2 weeks? 
  • Are you awaiting a COVID-19 test result? 
  • Do you feel ill or have any of the following symptoms? 
    • Fever 100.0 or higher
    • New cough
    • New shortness of breath
    • New sore throat
    • New chills
    • Sudden loss of taste or smell

 

If you answer “yes” to any of the questions listed, your appointment might need to be rescheduled.

 

 

During your visit

  • Currently no visitors are allowed to accompany you to your visit.  Exceptions include any patients that require assistance, but your visitor will also need to be screened upon arrival. 
  • When you arrive at the office, please ensure that you are wearing a mask.  
  • You will be stopped at the entrance for a pre-screening by a staff member.  
  • At that time your temperature will be taken.  
  • Once a normal temperature has been confirmed, you will be asked a series of COVID-19 related questions.  
  • Once cleared, you will either be escorted to the office (Guilford) or directed to meet our medical assistant on the first floor (North Haven). You will be brought directly to the exam room.
  • If you do not meet the safety guidelines, you will be escorted back to your vehicle and someone from our office will contact you for further instruction.  
  • We ask that you do not arrive too early as you will have to wait in your vehicle until your appointment time.  (If you are using a transportation service, please notify the driver that this will be the case.)
  • Please be aware that part of your visit might include a telephone interview with Dr Rath before she performs any physical exam.
